Before you book your first appointment, follow the steps below.

Tell us about YOU.

Complete our intake form so we can get to know you better. This includes providing your insurance information for verification and answering a few questions to help us tailor your treatment plan to your needs and preferences. Once submitted, our care team will begin reviewing your form.

Receive a Personalized Treatment Plan

After your intake form is submitted, our team will verify your insurance and review your responses. You’ll hear from us within 2-3 business days. We’ll reach out via email or phone to confirm your details and schedule your initial 60-minute appointment. During your first session, we’ll share a plan that’s tailored to your goals and preferences.

Continue Your Care

Together, we’ll create a plan for ongoing support. You can schedule follow-up sessions as needed for medication management, accountability, and check-ins. For your convenience, follow-up appointments can be scheduled through our online booking system or by contacting our care team. This is a partnership—we’re here to guide you on your path to success.

Fees & payment

Payments, including fees or co-pays, are due at the time of your session.

You can pay using any of the following options:

  • Health Savings Account (HSA)

  • Credit cards: American Express, Discover, MasterCard, Visa

  • Digital payments: Venmo, PayPal, Zelle

  • Cash

You will receive an email confirmation after your payment is processed. If you have questions about payment options, please contact our support team.

Cancellation policy

We understand that schedules can change. If you need to reschedule or cancel, please provide at least 48 hours' notice to avoid being charged for the session. We’ll do our best to accommodate your new scheduling request if the date and time are available.
